Health Certificate for Residence Application

All of the new foreign students must complete a health exam for a Resident Visa at a Ministry-designated hospital in their home country. They should submit the health certificate to the Healthcare Center on the registration day. 

Foreigner and Overseas students

The inspection items should include

1. Chest x-ray for tuberculosis
2. Stool examination for parasites
3. Serological test for syphilis
4. Proof of positive measles and rubella antibody titers or measles and rubella vaccination certificates
5. Download the Health Certificate for Residence Application
6. The list of authorized hospitals overseas

HK & Macau and Mainland China students

The school Division of Health Service will arrange for new foreign students the “Foreigner Health Examination and “Freshmen Health Check-up, both of which should be conducted in the designated hospital within 7 days after arrival.
Hong Kong and Macau students should have their vaccination record available for the physicians review at the time of the medical examination after arrival. If you dont have the record, the physician will work with you to determine which vaccinations you may need to meet the requirements.

Health exam for residence in Taiwan (Form B)	Proof of Positive Measles and Rubella Antibody or Measles and Rubella Vaccination Certificates (you may have your vaccination record available for the physicians review at the time of the medical examination after arrival. If you dont have one, the physician will work with you to determine which vaccinations you may need to meet the requirements.) Notes 1&2: The fee covers the medical exam items on Table 1, including the Measles and Rubella Antibody blood test fee. Note 3: The fee covers the medical exam items on Table 1, with the Measles and Rubella Vaccines administered. The hospital will issue the Measles and Rubella Vaccination Certificates. Note 4: With a Rubella Vaccination certificate, the Measles and Rubella Antibody testing fee is deducted. Note 5: If you do not have the certificate for Measles and Rubella Vaccination Certificates with the blood test showing negative antibody, you should be vaccine accordingly and acquire the Measles and Rubella vaccination certificate from the hospital. The price of vaccines is based on the annual report of the hospital.
