Health Certificate for Residence Application

All of the new foreign students must complete a health exam for a Resident Visa at a Ministry-designated hospital in their home country. They should submit the health certificate to the Healthcare Center on the registration day. For the regulations of the “Foreigner Health Examination,” please refer to the website Taiwan Centers for Disease Control.
Items required for health certificate: Download the heath exam form on the website of Taiwan Centers for Disease Control.
- Chest x-ray for tuberculosis
- Stool examination for parasites
- Serological test for syphilis
- Proof of positive measles and rubella antibody titers or measles and rubella vaccination certificates
- Examination for Hansen’s disease
HK&Macau and Mainland China students
The school Division of Health Service will arrange for new foreign students the “Foreigner Health Examination” and “Freshmen Health Check-up,” both of which should be conducted in the designated hospital within 7 days after arrival.
Hong Kong and Macau students should have their vaccination record available for the physician’s review at the time of the medical examination after arrival. If you don’t have the record, the physician will work with you to determine which vaccinations you may need to meet the requirements.
